This
is an AGREEMENT made and entered into this day with SUDDEN IMAGE and I
agree to engage the services of SUDDEN IMAGE for my event. I agree to
pay SUDDEN IMAGE the sum of 50% of Balance Due upon executing this
contract to insure my event date is reserved. I also agree to contact
SUDDEN IMAGE for payment arrangements. Balance is due 14 days prior to
event date unless written arrangements are made. Do You Agree?
You will qualify for a minimum of $100 down and a payment plan if
your event is over 6 months from today's date.

without consent and written notice of termination at least (6) weeks
